[#1 and #2 Caretaker]
Janeway left a fiancé in the Alpha Quadrant, Mark Johnson (his surname is revealed in [#83 Hunters]. When she left on the mission to find Tuvok and the missing Maquis ship in 2371 [#1 and #2 Caretaker] he looked after her pet Irish Setter Molly who was expecting puppies in seven weeks time. Mark is mentioned in several episodes. In [#24 Persistence Of Vision] Janeway's hallucination was in the form of Mark. But Mark gave her up for lost and told her in a letter she received in 2374 that he had married a co-worker [#83 Hunters].

Janeway had a brief interest in Kashyk, a telepath-hunter of the Devore Imperium [#104 Counterpoint].

She has no children apart from the offspring produced by mating with Tom Paris when she and he mutated into amphibious creatures after crossing the warp 10 barrier [#31 Threshold]. The offspring were left behind and the Doctor reversed the mutative process for both herself and Paris.

[Fair Haven]
In [#131 Fair Haven] Janeway fell in love with a holographic character called Michael Sullivan who inhabits Tom Paris' rural Irish holo-village Fair Haven. She altered his programming to make him more attractive; this included adding 3cm to his height, adding to his education and deleting his wife . Later she instructed the computer to deny her access to his program so that she could not alter him, and would have to accept him as he was, flaws and all.
In [#138 Spirit Folk] the Fair Haven characters malfunction and notice that Voyager crew, whom they once accepted as village residents, used the "unholy magic" of computers. The villagers set upon Paris, Kim and the Doctor, despite Sullivan's efforts to restrain them. In his search to learn more about Katie O'Clare, Sullivan transports to Voyager's bridge, where Janeway decides to tell him everything except that he is a hologram. The villagers of Fair Haven agree that although their world and Voyager's world are different they can resume their friendships.

In 2377, in [#162 and #163 Workforce], Janeway, along with the rest of the crew (except the Doctor, Chakotay, Neelix and Kim) are tricked into abandoning Voyager to avoid tetryon radiation. Having abandoned the ship in escape pods, they are 'rescued' by Dr Kadan and his associates. Kadan, a noted neuropathologist on the planet Quarra, alters the captured crewmembers' memories so that they forget their past lives and will be happy in their new ones. Because of the severe labour shortage in the region, and in order to gain personal profit for himself, Kadan sends them to work in the capital city, many crewmembers including Janeway working in the city's main power distribution plant. There Janeway meets Jaffen, another worker (who has not been subjected to Kadan's 'treatment') and they fall in love. She moves in with him. But Chakotay goes to Quarra, at first in disguise, and persuades her to trust him. After Chakotay is detained by Kadan, Janeway and Jaffen secretly contact Voyager and lower the shield grid round the planet so that Voyager can beam the crew back to the ship. Kadan's conspiracy is revealed to the authorities who, after restoring the minds of other similarly captured labourers, will repatriate them. Jaffen visits Janeway in her private quarters on Voyager where they say an emotional farewell. He returns to Quarra, where he has just been promoted, and Janeway returns to her real job - commanding Voyager.
