When Voyager attempted to travel all the way home in a quantum slipstream, in [#100 Timeless] she nearly reached the Alpha Quadrant but crashed in the Tekara sector, killing all aboard. Chakotay and Kim were the only survivors because they were aboard the Delta Flyer at the time, paving the way for Voyager.

From the wreckage Chakotay and Kim retrieved and reactivated the Doctor's program, along with Seven's frozen body.

The Doctor accessed information from Seven's implants to determine the exact time of the crash. With that information, and using a Borg temporal transmitter, Chakotay and Kim aimed to send the phase corrections to Voyager to allow her to reach home safely.

The Doctor refocussed Kim's effort after Kim's attempt to send phase corrections failed.
Kim: "They trusted me and I killed them! ... Don't you see, history's repeating itself. I destroyed Voyager once and I'm doing it again."
Doctor: "Somebody has to knuckle down and change history and that somebody is you." The Doctor suggested that if Kim cannot bring Voyager's crew safely home, he could at least send them a warning: "You told me you can't correct their phase variance. Alright, we have to accept that. But what about sending Voyager a warning? Is there a way to get them to abort the slipstream flight?"
Kim realised he can send a phase correction which will disperse the slipstream entirely - Voyager will not make it home but the crew's lives will be saved.

When power starts failing on the Delta Flyer, the Doctor willingly sacrifices his existence, in the form of the mobile emitter, as it has its own power supply, so that Kim can continue to work on the phase corrections. Accordingly, Kim was able to send phase corrections which shut down the slipstream drive: this saved the crew's lives but left all of Voyager's crew (including Chakotay, Kim and of course the Doctor) back in the Delta Quadrant.

On stardate 52479, and depicted in [#105 Latent Image], the Doctor discovered on Kim's body scar tissue from an operation which he did not remember performing.

 

He discovered that the memory files relating to a particular incident had been erased because his reaction to those events had caused him, in effect, to have a breakdown so that he was unable to function properly.

 

At first Janeway ordered those memory files to be deleted again once he learned about them, but he objected and was supported by Seven.

 

As a result he was allowed to learn what had happened in full: he had been aboard a shuttlecraft with Kim and a female ensign, Jetal, when they were attacked by an alien who fired upon them.

 

Kim and Jetal suffered severe injuries and at the triage stage, with both of them having an equal chance, the Doctor had to make the awful decision which person to treat and thus save, because there was not enough time to save both. He had treated Kim and Jetal had died.

 

Janeway helped the Doctor cope with the distress that his dilemma caused him by spending a lot of time with him (16 hours non-stop), and leaving her book of poetry for him to read which started with the words, "Here begins a new life." At last, the Doctor started to move on from the traumatic experience.
