Janeway goes to Sickbay. She says: "I can't do my job with the two of you constantly talking in my head."

She accesses the hidden Doctor's mobile emitter.....

.....and reverts to "her" normal appearance.

"She" turns out to be the Doctor!

In Sickbay, the Doctor reverts from masquerading as Janeway to his normal appearance.

Doctor: "No! I'm not doing anything else until I speak with the Captain. Fine - what's your encrytion frequency?" He types it into his portable computer.

It turns out that the two 'in his head' are Zet and Nar, two renegade members of the species known as the Hierarchy, who have taken Janeway captive aboard a stolen Hierarchy surveillance vessel.
Doctor: "Captain, are you alright?"

Janeway: "Doctor, listen to me. Under no circumstances are you to eject the warp core."
Doctor: "If I don't do what they ask they'll kill you."
Janeway: "I'm giving you a direct order. Stop what you're doing."
Doctor: "I'm sorry, Captain. I can't obey that order."
Janeway: "Doctor, I want you to go to the Bridge right now and alert Commander Tuvok."
Zet tells Nar: "Put her behind the forcefield."

Nar escorts Janeway to a small holding cell at the rear of the Bridge. As soon as she is inside Nar activates the forcefield.

Zet to the Doctor: "We have another assignment for you. I've been going over the schematics of your vessel, and I'm intrigued by this bio-neural circuitry. I want you to bring us a series of these gel-packs."
The Doctor protests: "The gel-packs are integrated into the ship's systems. I can't just remove them without someone noticing."
Zet: "You've proven yourself to be very resourceful, Doctor. You'll find a way. Remember, we're monitoring your perceptual subroutines. We can see and hear everything."

In Sickbay Chakotay's combadge signals and Tuvok's voice says: "Tuvok to Chakotay."

Zet warns the Doctor: "You know what will happen to her if you alert your crew."

Janeway looks on, helpless, as on one of the viewscreens Zet and Nar observe the Doctor's every move.
The Doctor answers Tuvok's signal mimicking Chakotay's voice, via comms. "Go ahead."
Tuvok, via comms: "Are you alright, Commander?"

Doctor, via comms: "Just a scratchy throat. The Doctor's treating me now."
Tuvok, via comms: "Could you come to the Shuttlebay when you're finished."
Doctor, via comms: "I'll be right there. Chakotay out."

After the communication ends the Doctor orders the computer: "Computer, access the Holodeck database and locate Commander Chakotay's holographic template. Download those parameters into my program." The computer does so.

Then the Doctor transforms himself into.....

.....Chakotay.

His disguise perfect, he puts on Chakotay's combadge, pulls down his sleeves and departs for the Shuttlebay.

Doctor/'Chakotay': "You wanted to see me?"
Tuvok: "I have completed the diagnostic as you requested. The Flyer's comm. system is functioning normally."

Doctor/'Chakotay': "Do you find that unusual?"
Tuvok: "The Captain did report that it was damaged during the attack by the R'Kaal."
Doctor/'Chakotay': "I'll have a talk with her. Did you find anything else out of the ordinary?"
Tuvok: "No."

Zet to the Doctor/'Chakotay': "Get out of there before you arouse suspicion!"
Doctor/'Chakotay': "I'll be on the Bridge if you need me." He leaves.

Nar: "You're doing a wonderful job, Doctor." Zet turns and stares at him. To Zet: "Just giving him a little encouragement."
Zet to Nar: "That's why I'm in command."
Zet and Nar go back to their duties.
Nar: "If all goes well, we should have the warp core in less than six hours. He's making excellent progress."

Janeway: "That's exactly what he wants you to believe."

Zet: "What do you mean?"
Janeway: "Did you really think you could outsmart a hologram?"

Nar: "Everything appears to be normal."

Janeway: "How do you know you're not looking at a holographic projection? The Doctor isn't just a physician. His emergency command subroutines contain thousands of tactical scenarios. I wouldn't be surprised if Voyager was already on its way here. You do have an option: leave me behind in an escape pod and I'll forget any of this ever happened."

Nar to Zet: "Maybe we should let her go."

Zet: "Are you delusional? It took months to plan this. We've already found a buyer for the warp core. Now you want to abandon everything?"

Nar: "It's better than being sent to a rehabilitation centre."

Zet: "We made our choice when we stole this vessel. I'm not going back to the Hierarchy and neither are you." Looking briefly back at Janeway: "She's just trying to trick you." He puts a hand on Nar's back in a friendly way: "A month from now, we'll be soaking in the mudbaths of Eblar Prime. Trust me."

In Janeway's quarters the Doctor uses Janeway's voice, via comms: "Janeway to Torres."
Torres, via comms: "Torres here."
Doctor (Janeway's voice): "I'm having a little trouble with my replicator again. Can you take a look at it?"
Torres, via comms: "I'm still working on those modifications you asked for. I'll send somebody else."
Doctor (Janeway's voice), via comms: "I'd consider it a personal favour if you'd do it yourself, Lieutenant."
Torres, via comms: "I'll be there in five minutes."
Doctor (Janeway's voice), via comms: "Thank you. Janeway out."

Doctor: "Computer, access Lieutenant Torres' holographic template and download the physical parameters into my program."

The computer does so and he transforms himself into Torres.

But there is an error - there is no sign of Torres' pregnancy! "Computer, access medical file Torres 3 and update her holographic template."

The computer does so and a pregnant "Torres" leaves Janeway's quarters.

The Doctor, masquerading as Torres, waits as Torres leaves engineering.

The Doctor/'Torres' enters Engineering.

Vorik: "Did you forget something, Lieutenant?"
Doctor/'Torres': "I did, actually. Where do we keep those spare gel-packs?"

Vorik: "Locker gamma 5."
Doctor/'Torres': "Of course, gamma 5."
Vorik: "On the upper level."

Doctor/'Torres': "I know where it is, Ensign."
