MEDICAL REPORT:
Voyager encounters a spatial rift which causes the warp core to destabilise. There, as Chakotay tries to maintain containment, a chronokinetic surge interacts with the warp core, strikes him unconscious and shatters the space-time continuum aboard the ship. Chakotay wakes in Sickbay. He learns that his body was in a state of temporal flux (he had the liver of an 80-year-old man and the kidneys of a 12-year-old boy), but that the Doctor created a chroniton-infused serum which restored him to normal. Chakotay asks the Doctor to come with him to check for other injured crewmen, but the hologram says he cannot leave Sickbay and he has never heard of a mobile emitter - Chakotay thus realises that the Doctor is from a time period before mid-2373.

It turns out Voyager has been fractured into 37 different timeframes, further supported when Chakotay encounters a 2371 era Janeway and Seska from early 2373. Chakotay discovers that he, unlike other people and objects, can pass safely between the timeframes, doubtless due to the serum. Chakotay returns to Sickbay and updates the Doctor who manufactures more serum, replicating a chroniton-infused hypospray casing using the same principles he used to make the serum.

Chakotay: "That serum you gave me, it must've made me immune. Normal technology won't pass through the barriers. Can you replicate a chronoton-infused hypospray casing using the same principles you used to make the serum?"
Chakotay kidnaps the 2371 era Janeway and injects her with the serum. They visit several other timeframes, discovering from the still Borgified Seven of Nine (from the
[#68 and #69 Scorpion] era, end of 2373 and early 2374) how to force the timeframes back into temporal sync, namely they must inject the bioneural circuitry (because it extends throughout the ship) with the Doctor's serum in order to transmit the necessary chroniton field.
Chakotay and the past Janeway reach Sickbay where, as the Doctor replicates more serum, he accidentally reveals to her that her crew will get stranded in the Delta Quadrant.

as Janeway gets kitted up with a supply of serum ampoules, the Doctor lets slip that the original (human) Doctor was killed in the incident that led to Voyager "being stranded in the Delta-" and Janeway later questions Chakotay, completing the Doctor's words as "Delta Quadrant"
As the pair travels through the ship administering the serum into the bioneural circuitry, they experience more timeframes from Voyager's past. They also briefly experience the timeframe from just after Chakotay's present, when they visit the Messhall which is acting as Sickbay. Paris orders a crewmember assisting him: "Keep dispensing the netrazine until we run out, then switch to the analeptics." He tells Chakotay that the patients there are suffering from radiation poisoning: "The EPS relays overloaded when we ran into the anomaly. We need the Doctor and medical supplies or we're going to have a lot more casualties." Chakotay orders him to cope as best he can. Janeway watches helplessly as Tuvok dies of the radiation. Eventually she, Chakotay and other crew from different timeframes enable Chakotay to bring Voyager into temporal sync and prevent the accident from occurring.
NOTE:
In the alternative 2377 timeline, Chakotay mysteriously describes Paris to 2371-era Janeway as "chief medic" without reference to the Doctor, his words being: "…..Or Tom Paris, a former convict who'll be our pilot, chief medic, and a husband to B'Elanna Torres."

To Chakotay: "Your body was in a state of temporal flux. You had the liver of an eighty-year-old man, and the kidneys of a twelve-year-old boy. Fortunately I was able to create a chronoton-infused serum that brought you back into temporal alignment. Anywhere else, that antidote would've earned me a prestigious award. Of course, on Voyager it's just another day in the life of an under-appreciated EMH."
