MEDICAL REPORT:
While on a planet collecting food supplies, Chakotay fetches Ensign Seska from within a cave where she claims she went searching for his favourite food, mushrooms, but he is surprised to find Kazon warriors there. One of them opens fire and injures him. The wound is treated in Sickbay. While he recuperates, Voyager receives a distress call from a Kazon-Nistrim ship. They find all the Kazon crew dead except for one, who is beamed to Sickbay. The Kazon ship was damaged by an explosion involving Federation technology, indicating a traitor on board Voyager. The injured Kazon warrior's body has been altered and fused with inorganic matter, as the Doctor reports: "His cells have mutated, or they've been fused with inorganic matter. I can't make heads or tails of it. Look at this. His bloodcells have bonded with metallic nuclei." The Doctor conducts a complete pyrocyte replacement to keep the Kazon warrior alive. Kes runs a cytological screening of the crew to look for a blood type match so that the patient's blood can be completely replaced. However, when his First Maje, named Culluh, visits, the Kazon patient is murdered to prevent him talking about the Federation technology; the murder is carried out by Culluh's aide injecting a rapid-action nerve toxin into the patient's system via a needle.

Seska, knowing she and Lieutenant Joe Carey are suspected of giving Federation technology to the Kazon, makes an unauthorised and dangerous visit to the stricken Kazon ship to retrieve evidence. Tuvok suspects her of trying to destroy evidence but Chakotay, her former lover, feels she is simply trying to clear her name. Before they can ascertain the truth, Seska is injured and is beamed directly to Sickbay. There the Doctor starts treatment, ordering Kes: "Prep 3 c.c.'s kelotane. I'll need a dermal regenerator. Run a full blood-scan."

Until she was injured, Seska had avoided giving a blood sample for the ship's medical records. After treating her injuries, the Doctor reports to Janeway that Seska's blood reveals that she is not Bajoran as per her appearance but actually Cardassian. Seska explains the blood chemistry discrepancy by saying she suffered Orkett's Disease (a terminal virus) as a child in the Bajoran work camps and would have died were it not for a bone marrow transplant from a sympathetic Cardassian woman. Unwittingly falling into a trap, Seska uses the computer system in Sickbay in order to bluff that Carey is the traitor, but this actually reveals to the crew that it is Seska who is the traitor. The Doctor is called online to testify that Seska's story about Orkett's Disease is incorrect, though he says that a less informed doctor might have been convinced.
