CARETAKER

MORGUE RECORD:
When Voyager is brought by a coherent tetryon beam (described by some sources as a displacement wave) into the Delta Quadrant, of the original crew complement of 141, over a dozen crewmembers are killed (the "over a dozen" figure is given by Ensign Kim in
[#154 Nightingale]), including the following: Lieutenant Commander Cavit the First Officer, Lieutenant Stadi the conn officer, astrogation plotter, transporter chief, the medical staff which certainly includes the ship's doctor who is the chief medical officer and also the nurse (comprising or merely including what Janeway refers to in
[#3 Parallax] as "medical support personnel"), and the chief engineer. Paris, upon him and Kim discovering the bodies of the doctor and the nurse: "They must've been right next to the console when it exploded."
(When Stadi mentions the crew complement of 141, presumably she excludes the Emergency Medical Holographic program (EMH), but it is not clear if she includes Tuvok, currently on assignment off Voyager and aboard Chakotay's missing Maquis ship, or Paris who has been temporarily assigned as an observer. But in
[#118 Relativity] the 2371-era Janeway says she began her mission into the Badlands with a crew of 153.)
MEDICAL REPORT:
Kim first activates the Emergency Medical Hologram (hereafter and throughout the site called "the Doctor") to carry out emergency treatment after the medical staff is killed by the Caretaker's displacement wave. He deals with several crewmembers for injuries sustained as a result of the displacement wave. For one of them he calls for 4 c.c.'s of trianoline, which is a pharmaceutical sometimes prescribed for percussive injuries, but has to fetch it himself. Another patient he tells after scanning them that they have no concussion and will be fine. Other patients vanish mid-treatment, and the Doctor will later learn that they were transported to the Caretaker's Array.

Presumably, after the Caretaker returns them, he begins to carry out medical check-ups of those patients. Later he treats Kes, a young Ocampan woman, for bruising and general physical mistreatment sustained at the hands of the Kazon-Ogla. Kim and Torres (the latter while not yet an official member of the crew) are treated after contracting a fatal disease arising from experimentation by the Caretaker entity although at one point they received Ocampan medication.
EMH SPECIFICATION:
The Doctor notes that his program is intended only as a short-term medical supplement although he is capable of treating any injury or disease.
MEDICAL ADMINISTRATION:
It is not until
[#4 Time And Again] that the Doctor learns that Kes, Neelix and the crew of another ship (a Maquis ship) have joined Voyager's crew.
DOCTOR'S LOG:
The Doctor confesses to Janeway in late 2377 in
[#170 Renaissance Man]: "I've had something on my conscience for a long time. After I was first activated, I kept a record of what I considered to be your most questionable command decisions. It's in my personal database. I hope you'll delete the file without reading it."
DOCTOR'S QUOTE:
As patients discharge themselves against his instructions to remain in Sickbay: "Is the crew always this difficult?"
